Depending on where you’re headed, it can be rather difficult to find certain medication. That’s why I always take some basic medication with me, enough to deal with a not-too-serious situation. Of course, I’m not a doctor and recommend you always ask your doctor for advice.

This is a list of medication I always take with me:
bandages (a whole box) and special bandages for blisters
disinfecting alcohol (I put some in a small plastic ‘travel’ bottle) and gel (e.g. Iso Betadine)
painkillers, e.g. Nurofen, Dafalgan, …
something against nausea
medication that stops diarrhea, e.g. Immodium
throat pastilles
clinical thermometer
contraceptive
For trips to exotic destinations, I also take:
bug spray
medication that prevents diarrhea, e.g. Enterol, to avoid diarrhea from eating exotic food
a 'general' antibiotic. Ask you doctor for advice.
other medication (e.g. enterol against malaria) depending on the destination. Ask your doctor for advice.
